As far as I know there are two different ones out there.
#31 from 1991 DC Cosmic Cards by Impel
and
the VS. Card (218??) from Upper Deck RPG Card
Does anyone else know of any others?

No. I got the Animal Man 1991's cosmic card, and I found it excellent (by the way, there's even a card dedicated to the Agents).

I have both, I think there's actually more Animal Man VS. cards after the Infinite Crisis boosters.

As far as I know, there is only one VS card. There is also a card from the DC Vertigo card set . I have the Cosmic Card as well, but I will have to find it to scan it.

That was sometime during the Vertigo run towards the end of the series. I wasn't a big fan of those issues.

They do reference the Vertigo era in the DC Encyclopedia.
I liked the Vertigo run. Nowhere near as good as the regular run, but I still liked it. (Well, all except the Prosser issues at the end.)
When Animal Man first re-appeared back into the regular DC universe, he still had the shaman/white hair look, and Maxine had her powers. When he next appeared much later, he was back to "normal".

Aquaman #35. It even has the Vertigo logo when it says Animal Man. I'll post a scan soon.
